The Stewardship Program has responsibility for long-term stewardship of lands with conservation easements held by the State of North Carolina. Monitoring entities have contracted to provide on-site inspections to ensure compliance with the conditions agreed to in the conservation easements.

Forms to be used in the monitoring process can be downloaded below. NOTE:
1. Save these blank forms as templates (.dot).
2. Save the completed forms as documents (.doc) with SPO File Number in the title, e.g. 101-010.101 Report.doc.

Send the completed forms as attachments to the Stewardship Program Administrator.
